POSITION SUMMARY:



The Population Health Coordinator works with internal and external contacts to organize the various components needed to initiate, run, and conclude a major project for the Population Health team. Duties include coordinating schedules and activities, placing orders for supplies and services, and tracking progress and results. Requires excellent communication skills and extensive knowledge of database and project management tools.



The Population Health Team serves as the central hub for programs that drive performance in the Medicaid ACO program. As part of the MassHealth effort to transform healthcare via formation of Medicaid ACOs, Boston Medical Center Health System has partnered with providers across the state to share risk in caring for Medicaid populations.



In order to succeed in this program, BMCHS designs and implements programs that improve quality of care for Medicaid populations, as well as manage utilization and total cost of care.



The Population health Coordinator is a critical member of the team who coordinates and supports this work. The Coordinator works with internal and external contacts to organize the various components needed to initiate, run and conclude a major project for the Population Health team. Duties include coordinating schedules and activities, supporting meeting preparation, tracking progress and results, and placing orders for supplies and services. Requires excellent communication skills and extensive knowledge of database and project management software.

ESSENTIAL DUTIES AND RESPONSIBLITIES :



1)
Outreach:






  • Conducts and coordinates timely, proactive patient outreach (telephonic, written, and in clinic) to schedule follow up appointments, coach patients as they prepare for appointments and/or routine screenings, and provide pre-visit reminders as needed. Follows up on patients who cancel or do not arrive to their appointments.
  • Work with Population Health Manager to review quality data to identify outreach needed.
  • Schedules outpatient visits and follow-up appointments working closely with internal and external resources to ensure patient is prepared to arrive to appointments.
  • Provides education, counsels, and supports clients. Uses a variety of culturally, linguistically and educationally appropriate strategies, in a variety of settings. Coordinates with Interpreter Services, as needed.
  • Collaborates and coordinates with health care team members, including social work and other resources, to promote positive prevention, harm reduction.
  • Ability to track and summarize outreach efforts for reporting purposes.
2)
Compliance and Contract reporting:



  • Documents patient encounters, resource development efforts, and other interventions for each patient, including date, time, and signature in the electronic medical record.
  • Complies with departmental, regulatory and contract requirements for documentation recording, and data collection, and submits department statistics and other records and reports within required time frames.

Meets hospital-wide standards in the following areas:







  • Conforms to hospital standards of performance and conduct, including those pertaining to patient rights and privacy.
  • Utilizes hospital's policies and standards as the basis for decision making and to support the hospital's mission and goals.
  • Follows established hospital infection control and safety procedures

Other Duties







  • Provides other administrative and special project assistance as required or assigned by the Population Health Manager.
  • Learns and understands Primary Care practice workflows with respect to optimal and coordinated health care for target patient populations.
  • Remains knowledgeable of and follows appropriate policies, procedures, and work standards, including but not limited to, policy regarding hours of work, absenteeism, and tardiness.
  • Maintains competency in areas of responsibility to ensure quality of care.
  • Maintains strict protocols of all confidential or sensitive information.
  • Attends hospital required and relevant training sessions or activities, as assigned by the Population Health Manager.
  • Promotes a positive and collaborative work environment supportive of the hospital's missions and goals.
  • Provides a link between the General Internal Medicine department and other hospital departments.
  • Adapts to changing departmental needs, including but not limited to, offering assistance to other team members, adjusting assignments, and other functions as determined by the Population Health Manager.
  • Assists in orientation/training of new staff members and volunteers.
  • Performs other duties assigned or as necessary.

EDUCATION:



Requires Bachelor's degree (or equivalent combination of formal education and experience).



Minimum 3-5 years of relevant post-collegiate professional experience required. Previous project coordination also preferred.




CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:



None.




EXPERIENCE:



At least 1+ year of experience working in a health care setting required.




KNOWLEDGE AND SKILLS:






  • Excellent verbal/written English communication skills, including excellent grammatical, editing and proofreading skills.
  • Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint, Access) and other web-based applications.
  • Ability to produce complex documents, perform analysis and maintain databases.
  • Ability to work independently and exercise independent judgment.
  • Excellent interpersonal skills necessary to be socially perceptive in accepting and relaying confidential information, communicating policies and procedures, and interacting effectively with a variety of Medical Center personnel and outside personnel.
  • Strong problem solving skills.
  • Proven ability to work with confidential information.
