Canyon Creek Behavioral Health:
Canyon Creek Behavioral is open and serving the behavioral health needs of adolescents and adults in Temple, Texas and all surrounding areas of Central Texas. Our behavioral health center will provide treatment options for individuals struggling with psychiatric health, chemical dependency and mood disorders such as anxiety and depression
Canyon Creek Behavioral Health is a 102-bed, acute care psychiatric hospital located in the beautiful Central Texas town of Temple, Texas. Canyon Creek features individual units for adolescents, adults, and seniors, and offers inpatient acute care and intensive outpatient programs. Qualifications
POSITION SUMMARY - DISCHARGE PLANNER
The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. The Discharge Planner works with patients to determine whether it is safe for them to leave the hospital and return home, as well as making sure future care needs are covered. The Discharge Planner works with Licensed Therapists in developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care
QUALIFICATIONS
Education: Bachelor’s Degree from an accredited college or university, in Social Work or a clinical related mental health field required
Experience: Minimum of one (1) year experience in a psychiatric health care facility with direct experience in discharge planning and/or case management, is preferred
Licensure: Not required
Additional Requirements: Crisis Prevention Certification Trainings (Handle with Care) within 30 days of employment and prior to assisting with a restraining procedure. Certification training is offered during new hire orientation
