Assesses the nutritional status and diabetes control of patients by utilizing diet, blood glucose records, exercise, medications and
other pertinent medical records as well as input from the patient. Employs pattern management to identify food patterns and
problem areas for individual patients.
Develops individualized plans of care and education for adults and children for self-management of medical nutrition needs,
cardiac, weight management and/or diabetes management. Educates and counsels adults and children regarding health,
nutrition, and basic physical activity practices that complement the lifestyle of the patient and family.
Develops measurable goals with the patient and family. Provides follow up medical nutritional/cardiac/diabetes counseling.
Works with staff to promote and expand outpatient nutrition services. Develops outpatient diabetes, cardiac, weight loss, and
MNT materials and resources. Oversees nutrition-related program planning. Provides nutrition education to allied health care
professionals, department staff and the community.
Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Minimum Required Education: Bachelor's degree in Food Science and Human Nutrition. Preferred: Master's degree in related field
Required Licensure/Certification: Registered Dietitian with the Commission on Dietetic Registration. Preferred: CDCES (Certified Diabetes Care and Education Specialist) as defined by the Certification Board for Diabetes Care and Education (CBDCE) or board certified in Advanced Diabetes Management (BC-ADM) by the Association of Diabetes Care and Education Specialists (ADCES).
Minimum Experience: 1 year experience as a Registered Dietitian.
The pay range for this position is: $31.61 - $47.41 / hour. Pay is dependent on applicant's relevant experience.An annual bonus may be paid to eligible employees based upon organizational and individual performance.
