The Discharge RN contacts discharged patients at home to ensure that they received effective discharge instructions. Provides information regarding any questions posed by the patients.
ADN, Diploma, BSN or Foreign EquivalentCurrent AZ State RN License in good standing
Two years nursing experience; strong knowledge in patient care and discharge planning RN
Excellent interpersonal communication skills; computer and general computer proficiency; awareness of system and community resources; strong team skills and ability to collaborate with other departments; ability to work independently.
