EEO/AA/Disability/Veteran Responsibilities RESPONSIBILITIES Leadership Provides daily supervision and clinical support to all members of group, including daily staffing. Meets with other leaders to develop goals to consistently advance and support the philosophy and goals of YNHHS Care Management Department and the health system. Provides modeling of professional behaviors at all times Takes opportunities to demonstrate visionary thinking, risk taking, and supports others to do the same. Sets example for accountability and responsibility of all daily activities and holds others accountable to the same. Is able to manage group dynamics, able to recognize when group is not functioning at capacity and develops plan/structure to improve teamwork. Demonstrates confidence in self and is open to feedback from all. Clinical Skills Remains clinically expert in area and provides clinical supervision in case management formally and informally. Demonstrates expert practice in daily activities. Remains aware of advances and trends in case management and applies them in practice. Demonstrates all core competencies of care management roles and responsibilities Participates in the orientation of new staff. Customer Satisfaction Demonstrates CHAMP and Standards of Professional Behavior Identifies patients, families, physicians and other health professionals as our customer and interacts in a proactive, responsible manner. RESPONSIBILITIES Fiscal Responsibility Prepare Kronos for manager approval. Remains aware of all payor issues and seeks creative resolution to problems by utilizing supervisory staff as needed. Makes staffing judgments based on fiscal goals. 5.Innovation Exercises interest and creativity while reviewing existing practices. Participate in projects and initiatives as requested by the manager. Brings issues forward for consideration after careful thought. Qualifications REQUIREMENTS EDUCATION (number of years and type required to perform the position duties): RN required. Minimum of Bachelors required in a clinically related field. Masters Preferred. EXPERIENCE (number of years and type required to meet an acceptable level of performance): At least four years as a Care Manager in excellent standing. Additional Information SPECIAL SKILLS: Leadership, critical thinking, understanding of group dynamics. ACCOUNTABILITY (how this position is held accountable for such as goals achievement, budget adherence, or other areas of accountability): Accountable for own practice and for the clinical supervision of care managers and transition coordinator staff.
