Case Manager/Outpatient - #689492

JOB REQUIREMENTS:

EDUCATION: Bachelor's Degree in Social Work or a health and human services related field. Qualified by education, training or experience to work with the adult and/or geriatric HIV/AIDS patient population.

EXPERIENCE: One (1) year relevant experience in direct delivery of case management services in a medical or community setting.

KNOWLEDGE, SKILLS & ABILITIES:

• Demonstrated administrative and organizational skills to meet job demands.
• Knowledge of HIV/AIDS disease processes and complications, demonstrated interpersonal, communication (written and verbal) and critical thinking skills required.
• Demonstrates ability to work both independently and in a team framework required. Must demonstrate respect for clients and maintain sensitivity to all issues and cultural awareness with patient population.

PHYSICAL DEMANDS: Frequent walking, driving and sitting. Exposure to infectious agents. Speaking, hearing and visual acuity is needed. Local travel is required. Occasional out-of-town travel is required.
