Overview:

Position Summary




The Student Nurse Practitioner (SNP), as part of the Clinical Operations Team, observes and demonstrates learning of clinical assessment and gap closure care for patients or health plan members in their homes and in facilities. All of the SNP activities are undertaken via oversight and guidance by the Nurse Practitioner (NP) Preceptor. The NP Preceptor will integrate clinical education and supervision to socialize the SNP in the role of the Matrix NP.





Will demonstrate the following learning goals:


  • Adult/geriatric assessment, medical history, physical exam, medication reconciliation, diagnosis, and health education
  • Patient education, communication to the PCP, follow-up facilitated with the PCP, and case management referrals may also be part of any assessment
  • Clear documentation according to Matrix guidelines and protocols
  • Matrix standards for quality, customer service, and teamwork through NP Preceptor guidance



Responsibilities:

Position Responsibilities


Observes and demonstrates the following:



  • Develop and share clinical learning needs and objectives with NP Preceptor and discuss strategy to meet them
  • Observe comprehensive health risk assessments on members assigned to NP Preceptor in member homes or facilities
  • Collaborate with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as appropriate
  • Identify quality improvement activities through assessing and planning health care individualized to the member and consistent with policies and protocols
  • Review and interpret the medical and social history of members
  • Observe and identify appropriate screening procedures for members
  • Interpret laboratory data, recommend appropriate follow-up, reconcile medications
  • Adhere to Matrix dress code and professional behavior guidelines
  • Adhere to all applicable laws, regulations and Matrix policies and procedures. Participate in all mandatory compliance and other training programs. Attend all scheduled member visits on time and prepared or notify NP Preceptor as soon as possible if unable to attend

Educational Requirements:



  • Bachelor of Science in Nursing preferred
  • Matriculated in accredited Master’s Degree or Doctorate of Nursing program with an emphasis in family, adult, or geriatrics

Licensure/Certification Requirements:



  • Current RN license in state of practice
  • Current CPR certification

  • Demonstrate effective written communication skills
  • Demonstrate effective verbal communication skills, including but not limited to speaking effectively to and listening to members, providers, families and staff

  • Is dedicated to meeting the expectations and requirements of preceptorship goals, and professional responsibility to members and NP Preceptor

Travel Requirements
:


  • Must meet company driving standards
  • Must have valid state driver license and be able to drive a car
  • Must provide proof of car insurance coverage
  • Must be able to independently travel to assigned locations



Physical and Psychological Demands:



  • All learning activities are voluntarily entered into by the student for the purpose of meeting their learning objectives
  • Requires prolonged sitting
  • Requires bending, stooping, twisting, kneeling, crouching, crawling and/or stretching from seated or standing positions
  • Requires hand-eye coordination and manual dexterity sufficient to operate medical equipment, computer tablet, computer keyboard, telephone, copier, calculator and other office equipment
  • Requires close vision, peripheral vision, and ability to adjust visual focus, hearing and smelling
  • Must be able to communicate information via telephone, computer or tablet
  • Requires moderate to intense concentration due to complexity of the position
  • Must be able to observe “Universal Precautions” for blood-borne pathogens and body fluids
